A closer look at electrical pulse relief
It is late afternoon. The laptop is still open, but your hand has drifted to the place where your neck meets your shoulders. You press the area, roll one shoulder, tilt your head—and the tight, dull ache soon returns.
Research suggests that TENS may reduce pain during or immediately after use for some adults. Why can neck muscles feel sore after a long day?

How does TENS work for neck pain?
TENS stands for transcutaneous electrical nerve stimulation. “Transcutaneous” means through the skin. A TENS device sends controlled, low-voltage pulses through contact points resting against the body. The sensation may feel like tingling, tapping, buzzing, or a pattern that rises and falls.
Those pulses create non-painful sensory input. One proposed explanation, often called gate-control theory, is that this input can reduce how strongly pain signals are transmitted through the spinal cord. TENS may also engage the body’s own pain-modulating pathways. The exact response varies from person to person.
This is different from mechanical massage. Nothing is rolling, pounding, or kneading the muscle. TENS works through sensory stimulation rather than pressure, which is why a light wearable device can create a noticeable sensation without squeezing the neck.
What does the research say about TENS for neck pain?
The largest modern review pooled 381 randomized trials involving 24,532 adults with many types of acute and chronic pain. Pain intensity was lower during or immediately after active TENS than after placebo, and the authors rated this finding as moderate-certainty evidence. The review also noted uncertainty around the size of the effect for individual pain conditions and around longer-term outcomes.
Research closer to the neck and shoulders is smaller but relevant. In a randomized study of 105 people with upper-trapezius myofascial trigger points, adding TENS to a programme of heat, movement, myofascial release, home exercise, and posture advice produced greater immediate and short-term improvements in pain and neck movement than the comparison programmes.
Another randomized trial followed 50 people with neck pain related to myofascial trigger points. Both the TENS and dry-needling groups improved in pain, neck-related disability, and cervical movement over 28 days. Dry needling reduced pain more in that study, and there was no sham group, so the result should not be treated as proof that TENS alone caused every improvement.
A placebo-controlled trial also found that ten minutes of burst-pattern TENS increased pressure-pain threshold over latent upper-trapezius trigger points more than sham treatment immediately after the session. The between-group effect was small.
These studies used specific clinical protocols. A consumer neck device is not identical to every machine used in a research clinic, and findings from one group of patients do not guarantee an individual result. Taken together, the evidence supports trying TENS as a temporary, noninvasive comfort option while keeping expectations measured.
What can TENS relief feel like?
TENS does not usually arrive as a dramatic before-and-after moment. It is more often a gradual change in the background. A steady buzz may compete with some of the dullness, or a tapping pattern may make the ache feel less prominent while the device is running.
That window can be useful. It may make it more comfortable to take a short walk, perform gentle exercises recommended by a physical therapist, move around the kitchen, or simply enjoy the evening without constantly rubbing your neck.

How strong should a TENS setting feel?
The best intensity is not the highest number on the device. It is the point where the pulse feels strong enough to notice but comfortable enough that your shoulders do not brace against it.
Start low. Give the pattern several seconds to become familiar, then move up one level at a time. A suitable setting may feel like clean tingling, rhythmic tapping, or buzzing. You should still be able to breathe normally and let your shoulders rest.
If the sensation becomes sharp, burning, or concentrated at one edge, turn the device off. Recheck its position on clean, dry, intact skin and follow the device-specific instructions before trying again.
Can heat and TENS be used together?
Heat and TENS create different sensations. Gentle warmth may feel relaxing when muscles seem stiff or overworked, while TENS provides sensory stimulation intended to change how strongly pain is perceived. Do not assume heat is right for every painful situation. Fresh injuries, visible swelling, skin irritation, reduced temperature sensation, or conditions affecting circulation may call for a different approach.
